We're looking for just the right person to join our team as Health and Wellness Coach Richmond. You will be committed to Making Lives Better for people after brain injury and stroke, collaborating as part of an integrated, multi-professional Coaching Resource Team in the development of individualized Life Redesign Plans for residents. You will manage the health and wellness aspects of a residents experience at CONNECT, participate in the mentoring and development of staff, contribute to our continuous learning program, and help us in maintaining CONNECTs accreditation status with Accreditation Canada. You will report directly to our Leader CONNECT Richmond.

What the role looks like. Within your scope of practice:
- Evaluate residents strengths, challenges, and lifestyle patterns, and their skills in managing their own health and wellness, and bring forward your ideas to be implemented by you, or by Life Redesign Coaches, as part of an integrated Life Redesign Plan;
- Ensure your services to individual residents are designed collaboratively, and in partnership with residents to achieve their goals, and to maximize their participation in real life, meaningful activities, equitable relationships and community;
- Support residents learning by showing other team members how they can best help/support the health and wellness process;
- Collaborate with other team members, integrating their techniques and strategies into the health & wellness process;
Skills and Abilities:
- Able to complete nursing assessments to identify residents health care needs and how to best support those needs through internal and/or external services.
- Able to delegate tasks to Life Redesign Coaches while providing continuous coaching, education, guidance and evaluation.
- Able to provide direction to Life Redesign Coaches regarding urgent resident health needs both during and after hours (as part of our emergency on-call team)
- Able to follow and implement physician directions and orders.
- Able to contribute to, and occasionally lead, resident care conferences with families, funding partners, and physicians.
- Able to communicate effectively with residents, families, Life Redesign Coaches, interdisciplinary team, physicians, and funding partners.
- Able to create, and contribute to, an atmosphere of safe, quality resident and family-centered care.
- Able to follow and implement Public Health infection prevention and control measures.
- Able to complete professional reports and assessments (InterRAI, WSBC reports, Braden scales, seizure protocols, serious incident reports etc.).
Job Types: Full-time, Part-time
Salary: $33.00-$41.00 per hour
